Healthy Thinking Institute. Outstanding in our fieldThe Healthy Thinking Institute Ltd was formed by Dr Tom Mulholland, a General Practitioner and e-health pioneer with a reputation as an innovator in e-health and mental health.

Dr Tom founded Doctor Global in 1998, delivering many on line consultations and designing a central medical record from which e-care for Moods was provided to Kaiser Permanente in the USA and Louisiana Health Care.
The Healthy Thinking concept was formed by Dr Tom when he suffered from depression after the breakdown of his marriage in 2000. Despite being a doctor and being put on medication he felt there had to be a better way to control his mood. He found Cognitive Behavioural Therapy (CBT) to be very effective and undertook training in Rational Emotive Behavioural Therapy (REBT).

Finding it difficult to access good treatment despite his knowledge and contacts, Dr Tom began to form an everyman's approach to Cognitive Behavioural Therapy and to accept speaking engagements on the topic. Recording mood data from over 5000 workers in businesses he addressed, Dr Tom, with the assistance of Graeme Beals, his mentor and publisher, wrote an international best seller on the topic, called Healthy Thinking® which is in 12 countries and has sold over 20,000 copies in New Zealand alone.

The book is recommended to patients by many GPs and Healthy Thinking® has successfully been used to treat a large number of patients with anxiety, depression and OCD using a CBT model.

Dr Tom went on to host his own daily TV show Dr Tom "The Attitude Doctor" which covered topics such as depression, refining the Healthy Thinking approach further before writing another successful follow up book; The Power of Healthy Thinking in which Attitude States (as opposed to traits) are discussed. This is also on the best seller list in countries like India.

In 2006 Dr Mulholland founded The Healthy Thinking Institute Ltd, with Graeme Beals becoming its first shareholder responsible for translating the Healthy Thinking concepts into educative materials. Graeme realised that Dr Tom was so busy doctoring, researching, speaking and writing that his valuable ideas would not ‘go global' without the use of modern educative technologies.

A former teacher and university lecturer, Graeme had become an educational publisher and business entrepreneur, He worked on the development of Dr Tom's teachings from his lectures, first into books, then into a one-day face to face course that could be delivered by trained professionals, then from that, into the substantial e-course which has so impressed many experts in the e-blended learning field.

The Healthy Thinking Institute began to form ‘faculty' relationships with an aim of further enhancing its teaching and research effectiveness. In 2007 Dr Mulholland was invited to join the Department of Psychological Medicine, University of Auckland Medical School, as an honorary lecturer and researcher in recognition of his work on depression, CBT and his research into workplace mental health.

Sharing offices with Professor Rob Kydd, Psychiatrist and Head of Department of Psychological Medicine, and Professor Keith Petrie, Clinical Psychologist and Professor of Health Psychology, and working alongside Malcolm Johnson has been a boon in ensuring that all teaching and research by the Institute is conducted to the highest ethical and research standards.

Dr Tom has now given hundreds of presentations on depression, mood, attitude, behaviour and productivity globally to corporations including the likes of Google, Microsoft, HP and Hilton, and to large numbers of physicians right through to community groups and many thousands of individual people have received help, all over the world, from the books and the e-course. Many letters are received that indicate that lives have been saved by transformation that Healthy Thinking can offer people.

Dr Tom Mulholland met Dr Paul Englert through research commissioned by the Institute, on the effectiveness of the Healthy Thinking teaching in a work setting. Dr Englert, an occupational psychologist and founder of OPRA Group Ltd, was so impressed with the outcome of the research, he bought into the Healthy Thinking Insititute and with Teresa MacGregor, began working on developing the attitude data Dr Mulholland had collected from over 5000 course attendees, into the first psychometric test for attitude.

The Healthy Thinking Institute Ltd sets lofty goals that include a strong element of charity work conducted through The Healthy Thinking Foundation.

What Is Healthy Thinking?

Healthy Thinking's several thousand clients have taken part in workshops (physical and on line) identifying common symptoms such as stress, anger and frustration. These have been related to situations and triggers which generate a number of thoughts. Based on CBT and Rational Emotive Behavioural Therapy (REBT) patients are taught a simple technique we call Emotional Algebra® that they can apply whenever they feel an alarm emotion, to identify the trigger, and then the thought that they generated in response to it. They are taught techniques for reworking this thought to a more helpful one, and given a ‘language' to do it with.

Over years of online testing, a number of patterns and core beliefs have been identified, that tend to characterize the natural brain-language individuals fall into when self-talking in response to a trigger. Each person uses a different mix but there are 10 core types. Most people utilise a mix of two or three of these.

These patterns and core beliefs (attitudes) have been given easily identifiable profiles and names, and we have developed a way to counteract them, with Healthy Thinking® ‘cure' profiles. Multi choice ipsative (measuring yourself against yourself - eg a choice you make now compared with one you made previously) profiling tools are used so participants can identify their predominant ‘unhealthy thinking' attitude patterns and change them to more healthy patterns.

The combination of personal history, CBT techniques, profiling, diaries and exercises makes Healthy Thinking® a unique proposition. Its ability to be delivered in a standardized format with repetitive dosing as a "cogniceutical" will revolutionize the way we look at and treat mental illness. Our research (over 6000 people) shows that that there is significant morbidity and mortality in the workplace, with individuals on average claiming that they are experiencing over the most recent month at home and work;

Why Use Healthy Thinking?

Unhealthy emotions and behaviors are a direct result of unhealthy thinking. As Dr Mulholland describes it, this can be due to a combination of software (mind) and hardware problems (brain).

An independent research company; Occupational Psychology Research Associates (OPRA) was approached by the Healthy Thinking Institute www.healthythinking.biz in 2007 to independently evaluate the possible benefits of the corporate Healthy Thinking® program a year after the course was run.

Our Research

Brain management concepts from a medico-psychological perspective

There are two major and separate neurological pathways for mood. One is primarily affected by pharmaceuticals such as SSRIs while the other pathway is more affected by Cognition (World Congress of Neuropsychiatry Proceedings: Sydney 2006). In patient terms we often explain that mood can be controlled by the mind (software) and brain (hardware). Just as it is hard to run good software on corrupt hardware, a holistic view of mental health is taught by the Healthy Thinking Institute - ie that good hardware and good software are ideal. Factors such as nutrition, sleep, work-life balance, family history of depression can all affect neurotransmission and the ability to control mood as can rampant emotionality and poor attitudes.

CBT (Cognitive Behavioural Therapy - Healthy Thinking could be said to be an original approach to this and has been designed with CBT research in mind) can be used alone in mild to moderate forms of depression. CBT and Healthy Thinking® have been shown to have long lasting effects, well beyond the half life of pharmaceuticals.
Dr Tom Mulholland has coined the word Cogniceutical© to describe Therapeutic Thoughts which can be administered by e-therapy and electronic messaging.

Healthy Thinking® has been shown to be effective, with 92% of people surveyed finding Healthy Thinking® very effective or effective at home and no one thought it had not been effective in improving mood and relationships. Of those independently evaluated by psychology company OPRA Group, 75% of people thought Healthy Thinking® had created long term behavioural change at work. E-Therapy gives a perfect platform for standardisation of treatment and monitoring of results. It will provide a research tool to understand incidence, consistency and benchmarking of diagnosis. This will lead in to further development of efficacious ‘cogniceuticals' which lower the burden of depression and other mental health disorders.
Concept

If hardware (brain and chemical factors) is treated pharmaceutically by General Practitioners or Psychiatrists, Cognitive behavioural Therapy is more effective in combination. CBT can also be used alone in mild to moderate forms of depression. CBT and Healthy Thinking® have been shown to have long lasting effects, well beyond the half life of pharmaceuticals.

He has designed HAPPI© (Heart Attack Prevention though Phone Intervention - see Appendix 1) with Healthphone, the designers of STOMP (Stop Smoking Over Mobile Phones- see Appendix 2) which reduced smoking by half in a clinical trial.

General Practitioners face a number of challenges in treating depression, anxiety and other disorders such as OCD and addiction. These include factors such as;

  1. Lack of Exposure to CBT. Many GPs have never been taught or are unaware of basic CBT as a treatment for depression. Therefore when faced with a depressed patient, they are more likely to write a prescription for an SSRI, or refer the patient to a psychologist or "counselling", rather than initiate any form of CBT.
  2. Resources. Teaching CBT takes time which busy GPs often don't have. It takes at least 20 minutes to get the basics of CBT across to a patient, and that is after the diagnosis is made. There are few simple but effective CBT stories that patients find useful. Often books are more likely to cure insomnia than depression.
  3. Lack of diagnostic tools or tests for depression that are easily interpreted by patients.
  4. Lack of consistency of CBT providers and other counsellors and mechanisms for feedback.
  5. Lack of measurement of compliance.
  6. Lack of good data on incidence of depression and response to standardised treatments.

Dr Mulholland and his research team into ‘cogniceuticals' believe that CBT based e-therapy should be run similarly to that of a drug or Pharmaceutical Trial.

Following an e-diagnostic on-line questionnaire and assessment tool for depression and core beliefs (attitude states), patients receive a loading dose of a book (bibliotherapy) which has been found to be effective in hundreds of patients. This can also be delivered in the form of an e-book, DVD or CD Rom if necessary.
Further treatment doses of e-therapy are delivered via on-line participation and exercises.

Maintenance doses of the ‘Cogniceutical' are delivered via SMS text message and are customised based on the patient's core beliefs and attitude profile.

Compliance is checked and therapeutic effects of each on-line or text intervention measured by feedback on the website and return SMS. This enables researchers to assess the most efficacious ‘cogniceuticals' and to determine if there are in fact any non-therapeutic messages.
Dr Mulholland firmly believes in e-therapy for a number of reasons.

Standardised assessment tools of mood

  • Immediacy of therapy
  • Explanation and destigmatisation of condition
  • Possibility of on-line help.

To put this in real terms, Patient A goes to see GP Z who diagnoses depression based on history. No diagnostic tool is used. An SSRI is prescribed and the patient referred to psychologist T. The waiting time is 2 weeks and the patient has little immediate follow up. Psychologist T delivers a variant of CBT and Gestalt therapy and may write a letter to the GP. Success of treatment is not recorded.

Patient B could have seen GP Y who took a different approach. No SSRI, referral to a different counsellor and some ST John's Wort and Hypnosis.

1. Healthy thinking on line e-development program. This is a major undertaking and has been well received thus far by the market.   • Reminiscence in The Elderly - a case control study I designed matching rest home and domiciled people in their own home to look at the difference Reminiscing made to their adaptation to old age. As a 3rd year medical student with Professor John Campbell as my supervisor. Won the New Zealand Essay prize in Geriatrics.
  • Suicide: Is it Preventable or Predictable - 5th year medical student project - 10 year review of suicides that had been seen by the Crisis team in Christchurch, looking for any warning signs that may have predicted or prevented suicide.

Graduate

  • Diploma of Sports medicine - reviewed Head Injury in Rugby Players with and without headgear.
  • Healthy Thinking institute - collected on line data from approximately 2000 individuals answering qualitative and quantitative questions around attitude, behaviour emotions and stress at home and in the workplace. Formed the basis of the 10 Attitude profiles with Antidotes.

Graeme Beals B Ed (Hons), Dip Tchng, TTC

Graeme was a successful New Zealand school teacher, principal, Teachers College Lecturer and Director of a regional Teachers Centre (In-service Training and resource provider) before setting up an independent educational publishing company in New Zealand (Curriculum Concepts). That company has grown to also include e-delivered education as well as print and general publishing under the name Zenith Publishing Group Ltd, though it is still privately owned by Graeme and his wife Jane, who now runs the company.

Graeme has since been part of two educational IT companies, developed a leading-edge web-based self-publishing business and been General Manager of Distance Education for one of New Zealand's largest private distance educators, developing the distance delivery approach and helping to write the national plan that has now been adopted as the standard for NZ Government funded tertiary distance learning. In that role Graeme was also responsible for the development of a series' of learning CD Roms for tertiary level industry training, that were so successful with learners, 75% of whom were Māori, that doubts were raised as to whether it was possible for people to learn so fast.

Graeme also established and Directs his own finance company, Aspire Finance Ltd and is a significant shareholder and director in a number of syndicated property companies. He assisted Dr Mulholland to write his books and has now joined The Healthy Thinking Institute as a 10% shareholder - a working co-owner, with special responsibility for management, written communications and e-learning methodology.

DR Paul Englert - Opra Group Ltd

DR Paul Englert is a founding Director of OPRA Group Ltd. Paul is a registered psychologist, an ex-chair of the New Zealand Psychological Society I/O division, a supervisor for the psychologist board and was awarded a certificate of appreciation from the New Zealand Psychological Society in 2007 for services to New Zealand Psychology. Paul has a PhD in Psychology awarded for his work with career counselling for the young unemployed. Paul also has a first class honours (BSC) in industrial and organisation, an arts degree majoring in criminology, and a graduate diploma in applied statistics, specialising in evaluation (through Claremount University).

Paul has extensive experience as both an industrial psychologist as well as a youth counsellor. Paul began his career as a youth worker, specializing in working with ‘kids at risk' and juvenile justice. During this time Paul also worked as a telephone counsellor for youthline. As an I/O psychologist Paul and OPRA have completed a range of work related to the current RFP. This includes the creation of online diagnostics through to development of training courses and major evaluation projects. Paul has 14 published articles, is a sought after speaker and appeared on television shows such as Telstra business, Close up, and the psychologist for Target.

Professor Rob Kydd MB ChB, PhD, FRANZCP

Professor of Psychiatry, Head of the Department of Psychological Medicine, Faculty of Medical and Health Sciences, University of Auckland was appointed Professor and Head of the Department of Psychiatry and Behavioural Science in 1996. During 1998-1999 he was also Clinical Director of Mental Health Services for Counties Manukau District Health Board and was Chair of the New Zealand Branch of the Royal Australian and New Zealand College of Psychiatrists over the same period.  In 2000 he became Head of the School of Biomedical & Health Sciences and over 2002-2004 acted as Head of the School of Population Health.  He was Deputy Dean of the Faculty from 2002 to 2005 before returning to the Department of Psychological Medicine.

 Rob's research interests have ranged from basic neuro-physiological work on the effect of catecholamine neurotransmitters on the electroencephalogram (EEG) to health services research on the needs of long-term psychiatric patients living in the community.  More recently he has developed an interest in the role of the workplace in the prevention and management of illness particularly mental illness and has been providing clinical services  and opinions to employers and insurers through the occupational medicine clinics at the Tamaki Campus.

Dr Libardo Dario Suarez Carvajal - IT consultant and Developer

Libardo trained as a Medical Doctor and Surgeon at the National University of Colombia in Bogota in 1989, later emigrating to New Zealand where he has worked in medically related IT since 1999, first with Doctor Global developing the Electronic Health Record project and on e-Care for Moods: a Web based application developed for Kaiser Permanente. He also worked on a project for Louisiana Health Care using the e-Care for Moods core functionality, with some customised UI and functionality then on HTN and Diabetes Chronic Disease Management Software: a Web based application developed for Kaiser Permanente (USA Health provider based in California). Libardo's has also been working for the IMF (International Medical Fund) on a collection of Health Insurance applications data and its management.

Libardo's work has included hardware and software architectural decisions, client requirements gathering and documentation, general and technical scope, technical documentation(data flows, technical help documentation);data base (SQL server) design, developing, implementation and setting of maintenance plans(backups), programming, User Interface design, developing and maintenance (XSLT, HTML, ASP, ASP .Net). Application and Data base testing and tuning of alpha, beta and production versions.
